 
Evaluation
The Fairwinds eating disorders program begins with a bio-psycho-social evaluation and design of an individualized treatment program. The evaluation entails the questionnaires for the Eating Disorders International Database, which includes the Eating Disorder Questionnaire, EDE-4, EDI-2 and SF 36. Clients also complete the Beck Depression Inventory (BDI), the Yale-Brown-Cornell Eating Disorder Scale (YBC-EDS), the Bond Defense Style Questionnaire (DSQ) and the Guilt Questionnaire. In addition, a complete psychiatric history, including educational assessment for adolescent development history, mental status examination, medical history, physical examination and anthropometric evaluation, is obtained.
